DCLAD (Child) professional questionnaire scoring step 1 - child's chronological age


The first step in scoring the professional questionnaire is to establish the child's chronological age. This is important because the questionnaire is used to determine whether the child is able to function at the appropriate level for their chronological age.


The following table explains how to calculate the child's chronological age for the professional questionnaire.

If the questionnaire has been… Then the child's age in years on that date is established…
signed and dated, at that date.
amended, or a new questionnaire completed, at the date the new material was signed and dated.
completed but not dated, at the date of lodgement of the questionnaire.
